新生儿窒息后尿微量蛋白检测的临床意义 被引量:4

摘要 目的探讨尿β2微球蛋白(β2-MG)和白蛋白(Alb)对新生儿窒息早期肾损害中的诊断价值。方法取笔者所在医院2005年6月~2007年9月期间收治入院的窒息后新生儿48例,设为实验组,在入院后24~36h内采集尿液,以免疫投射比浊法检测尿微量蛋白;取同期无窒息及严重感染性疾病的住院患儿40例,设为对照组,进行上述检测。所有病例在获得尿液同时取静脉血分别查Cr、BUN对照。结果发现实验组尿β2-MG、Alb异常发生率明显高于对照组,差异有显著性(P<0.01)。且β2-MG、Alb异常发生率呈:重度窒息组>轻度窒息组>对照组,差异有统计学意义(P<0.01)。结论尿β2微球蛋白(β2-MG)和Alb是检测肾小球功能的特异性指标之一,并与窒息程度密切相关,对窒息引起的早期肾损害诊断具有临床实用价值。 Objective To evaluate the urinary β2-microglobulin(β2-MG) and albumin (Alb) early neonatal asphyxia in the diagnosis of early renal damage.Methods From June 2005 to September 2007,during this time admitted to 48 cases of neonatal asphyxia were designed to the test group,after admission within the 24~36 h the collection of urine was used to detect urine protein buy immune turbidimetry projection,took over the same period without severe asphyxia and infectious diseases,hospitalized children,40 cases as control group,to carry out such testing.In all cases venous blood were simultaneously obtained urine check Cr,BUN as control.Results The urinary β2-MG and Alb abnormalities of test group,were significantly higher,the difference was significant(P〈0.01).The incidence abnormalities of β2-MG and Alb:severe asphyxia groupmild asphyxia groupcontrol group, significantly(P〈0.01).Conclusions Urinary β2-microglobulin(β2-MG) and Alb is a specific marker for detection of glomerular function,and is closely related with the degree of asphyxia,has clinical value to diagnosis the early kidney damage which caused by suffocation.
