摘要
目的探讨应用能量多普勒超声(PDI)与血管内皮因子定量检测乳腺恶性肿瘤血流的相关性并评价其临床应用价值。方法采用PDI观察肿块血流信号的情况,将血流信号分为0-Ⅲ级,使用频谱多普勒测定肿块血流的峰值流速(Vmax)和阻力指数(RI),通过计算机辅助定量测定肿块彩色像素密度(CPD);术后标本组织进行抗VEGF免疫组化染色,判定血管内皮生长因子(VEGF)阳性表达率,得出结果与超声彩色像素密度值进行对比分析。结果能量多普勒显示恶性肿块的彩色血流信号丰富,血流分级较良性组级别高。彩色像素密度在恶性组与对照组间差异有统计学意义,恶性组并腋窝LN(+)的彩色像素密度高于腋窝LN(-)组,能量多普勒超声测定乳腺恶性肿瘤的彩色像素密度与VEGF表达(Expression)有较好的正相关性(r=0.9162);恶性组并腋窝LN(+)的血管内皮生长因子阳性表达率高于腋窝LN(-)组。结论乳腺肿瘤内的彩色多普勒能量成像血流丰富程度和VEGF从不同角度反映了乳腺肿瘤血管生成特性,PDI所检测的乳腺肿瘤内血流信号与VEGF表达相关性较好;术前采用PDI定量检测乳腺恶性肿瘤血供情况,可为本病的临床诊治及预后判断提供重要信息。
Objective To investigate blood average density of breast malignant tumor and the relevance of vascular endothelial growth factor(VEGF) with the quota examination of the power Doppler imaging(PDI).Methods The blood stream signal was observed in the tumor with PDI,and the blood flow signal was divided into 0-3 levels.The peak value speed of the tumor blood flow(Vmax) and the resistance index(RI) were determined by using frequency spectrum Doppler,and tumor color pixel density(CPD) was obtained by computer auxiliary quantitative determination.Masculine expression rate of VEGF was determined by immunohistochemical method.The contrastive analysis was performed between masculine expression rate and the supersonic CPD.Results PDI demonstrated that colored blood flow signal(BFS) was rich in the malignant tumor and the BFS' level was higher in malignant tumor than that in benign group.There was remarkable significance in CPD between malignant group and control group.The CPD of the malignant group with the armpit LN(+) were higher than that in armpit LN(-).The PDI determination of CPD and VEGF expression in the breast malignant tumor and there was good relevance(r=0.9162).The VEGF masculine gender expression rate of the malignant group with the armpit LN(+) was higher than that in armpit LN(-) group.Conclusion The degree of PDI blood flow and VEGF can reflect breast tumor vessel production characteristic in breast tumor.With the quota examination of the PDI blood flow before operation,it may provide the valuable information for clinical diagnosis and the prognosis in breast malignant tumor.
