彩色多普勒超声诊断单绒毛膜囊双胎特有畸形的诊断价值

Dignosis of special deformity in single amniotic sac double foetuses by color Doppler ultrasound

目的探讨彩色多普勒超声对单绒毛膜囊双胎特有畸形的超声诊断价值及临床意义。方法分析204例单绒毛膜囊双胎特有畸形的病历资料,总结彩色多普勒超声声像图表现,随访产后、引产后结果。结果 204例双胎妊娠孕妇中共检出16例单绒毛膜囊双胎特有畸形,其中联体无脑双胎1例,无心畸胎序列征6例,双胎输血综合征9例。14例经产前彩色多普勒超声明确诊断,漏诊2例为无心畸胎序列征,经引产后临床确诊。所有病例均经产后及引产后证实。结论彩色多普勒超声是产前诊断单绒毛膜囊双胎特有畸形的重要方法,可为胎儿预后判断及临床治疗提供重要依据。早孕期超声检查对双胎绒毛膜性的判断有助于单绒毛膜囊双胎特有畸形的诊断。

Objective To investigate the ultrasound diagnosis value or clinical significance in the analysis of special deformity of single amniotic sac double fetuses by color Doppler ultrasound.Methods 204 cases twin pregnancy women who came to our hospital before production were examined by color Doppler ultrasound,and the case data,and follow-up results of reproduction or induced labor were analyzed.And then the special characteristic graphs of color Doppler ultrasound was concluded.Results In 204 cases,there were 142 double chorion sac double foetuses casea,62 single amniotic sac double foetuses cases.16 cases who were single amniotic double foetuses were checked as special deformity,one of them was conjoined mindless twins,6 of them were acardia,9 of them were twin blood transfusion syndrome after clinical confirm.There 2 cases of acardia were missed diagnosis.Conclusion It is an important form that diagnosis of special deformity in single amniotic sac double foetuses before production by color Doppler ultrasound,it can provide significant basis to clinical treatment or prognosis judgment for fetus.
