保守治疗肩峰下撞击综合征30例

Conservative Therapy in 30 Patients with Shoulder Impingement Syndrome

目的探讨长效皮质激素局封+复方南星止痛膏外用与COX-2抑制剂治疗肩峰下撞击综合征的优缺点。方法将肩峰下撞击综合征55例患者按就诊顺序随机分为2组,治疗组(32例)采用复方倍他米松+利多卡因局部封闭、外贴复方南星止痛膏,对照组25例口服塞米昔布胶囊。治疗1个月后观察疗效。结果以VAS和ROM作为评价对比标准,结果用t检验方法作统计学处理后显示长效皮质激素局封+复方南星止痛膏外用组的疗效明显优于口服COX-2抑制剂组。结论保守治疗对于肩峰下撞击综合征Ⅰ、Ⅱ期患者的疗效比较满意,使用长效皮质激素局封+复方南星止痛膏外用并结合有效的康复指导能获得更为确切的治疗效果。

OBJECTIVE To explore the advantages and disadvantages of application of long term corticosteroid with blockade needle ＋ Compoud Nanxing Zhitong Paste with COX-2 inhibitor to treat shoulder impingement syndrome.METHODS 55 patients with shoulder impingement syndrome were randomly divided into treatment group and control group.Patients in treatment were treated with Compound Betamethasone＋ Iidocaine with Compoud Nanxing Zhitong Paste;patients in control group were administrated with Celebrex Capsule for one month.RESULTS VAS and ROM were used as comparison standard for evaluation.With the test,it was obvious that the effects of treatment were better those in control group.CONCLUSION Conservative therapy has satisfying effects in treating patients with shoulder impingement syndrome in phrase I and II.To use long term corticosteroid with blockade needle and Compoud Nanxing Zhitong Paste,together with effective rehabilitation guidance can achieve better therapeutic effects.
