背带式缝合术在剖宫产产后出血中的临床应用

Clinical application of B-Lynch suture on postpartum hemorrhage in cesarean section

目的 探讨子宫背带式缝合术用于治疗孕妇剖宫产产后出血中的临床应用意义.方法 将医院妇产科收治的产后出血患者48例,随机分为2组,实验组采用子宫背带式缝合术治疗,对照组行子宫次全切除术或者子宫全切除术.观察实验组和对照组的产后平均出血量、手术时间及接受输血治疗的发生率.术后对每例患者进行随访,测定患者的体温,计算每组患者的产褥病率.结果 在治疗孕妇乏力性子宫出血中,背带式缝合术在手术时间、产后出血量及接受输血治疗上明显优于对照组,差异具有统计学意义（t值分别为1.84、3.29、5.48,均P〈0.05）.术后随访,实验组患者产褥病率明显低于对照组,差异具有统计学意义（t＝5.16,P〈0.05）.术后一天对两组患者进行体温测量,实验组平均体温低于对照组,差异具有统计学意义（t=8.49,P〈0.05）.结论 子宫背带式缝合术是治疗剖宫产产后出血行之有效的止血方法,值得临床推广使用.

Objective To investigate the significance of clinical application of B-Lynch suture on postpartum hemorrhage in cesarean section. Methods 48 patients with postpartum hemorrhage accepted in department of gynecology and obstetrics of the hospital were randomly divided into two groups, experimental group treated with B-Lynch suture and control group with subtotal hysterectomy or hysterectomy. The average amount of postpartum bleeding, operation length and the incidence of receiving blood transfusion in both groups were observed. Each patient was followed up after operation, measuring their body temperature, and puerperal infection in each group was calculated. Results In treating postpartum hemorrhage operation length, amount of postpartum bleeding and receiving blood transfusion were all superior in experimental group than those in control group, and the differences were statistically significant （t was 1.84, 3.29, 5.48, all P 〈 0.05 ）. In following-up the incidence of puerperal infection in experimental group was remarkably lower than that in control group, and the difference was significant （ t = 5.16 ,P 〈 0.05 ）. The average body temperature of patients measured one day after operation in experimental group was lower than that in control group, and the difference was statistically significant （ t = 8.49, P 〈 0. 05 ）. Conclusion B-Lynch suture is an effective method to treat postpartum hemorrhage in cesarean section, and it is worthy of applying in clinics.
