血清β-hCG和孕酮检测对早期异位妊娠的诊断价值

Diagnosis value of β-hCG and progestone on early ectopic pregnancy

目的 探讨血清β-人绒毛膜促性腺激素（β-hCG）和孕酮检测对早期异位妊娠的诊断价值.方法 检测2005年2月～2010年2月期间经手术后病理或阴道彩超证实且停经〈8周的120例异位妊娠患者间隔48小时血清β-hCG和孕酮水平,选择同期同孕龄130例早孕妇女作为对照进行统计分析.结果 早孕组血清β-hCG平均值及48小时上升幅度均明显高于异位妊娠组（t值分别为2.35、4.22,均P〈0.01）.早孕组血孕酮平均值明显高于异位妊娠组（t=2.41,P〈0.01）.将血孕酮值8.97ng/mL作为临界值诊断异位妊娠,其敏感性达84.93%,特异性达90.36%,阳性预测值达90.34%,阴性预测值达88.13%.结论 检测血清β-hCG和孕酮可以明显提高早期异位妊娠的诊断率.

Objective To explore the diagnosis value of β-hCG and progestone on early ectopic pregnancy. Methods From February 2005 to February 2010, 120 cases of ectopic pregnancy within 8 gestational weeks which were confirmed by transvaginal sonography or pathological results after operations were performed blood tests of β-hCG and progestone level, and repeated the tests at an interval of 48 hours. 130 cases with same pregnancy age in the same period were selected in control group for statistical analysis. Results Both the average value of blood β-hCG and its rising range in 48 hours in early pregnancy cases were significantly higher than those in ectopic pregnancy cases （t was 2.35 and 4.22 respectively, P 〈0. 01 ）. The average value of blood progestone in early pregnancy cases was significantly higher than that in ectopic pregnancy cases （ t = 2.41, P 〈 0. 01 ）. If 8.97ng/mL were regarded as critical value of blood progestone to ectopic pregnancy, its sensitivity would be 84.93%, specificity 90. 36%, positive predictive value 90.34% and negative predictive value 88.13%. Conclusion Testing blood β-hCG and progestone can enhance the diagnosis rate of early ectopic pregnancy.
