慢性乙型重型肝炎患者血清CA125、CA199联合检测的临床意义

The Significance of Combined Measurement of Serum CA125 and CA199 in the Early Diagnosis in Patients with Chronic Severe Hepatitis

目的观察慢性乙型重型肝炎患者血清CA125、CA199的变化,探讨二者联合检测对慢性乙型重型肝炎的临床意义。方法随机选取慢性乙型肝炎轻型50例和慢性重型肝炎50例,两组患者留取血清分别用化学发光免疫法检测CA125、CA199含量,同时随机选取50例健康体检正常人作为对照组,对结果进行比较。结果慢性乙型重型肝炎患者血清CA125、CA199水平均显著高于健康对照组和轻度患者组,差异有统计学意义(P<0.01);轻度患者组与健康对照组相比差异无统计学意义(P>0.05)。CA125、CA199单独检测慢性乙型重型肝炎的灵敏度分别为62.0%、66.0%。二者联合检测慢性乙型重型肝炎的灵敏度为84.0%。两项联合检测与单独检测相比灵敏度明显提高(P<0.05)。结论联合检测血清糖链抗原CA125、CA199对诊断慢性乙型重型肝炎的灵敏度有较大幅度的提高,虽不能作为乙型重型肝炎的特异性指标,但对临床早期辅助诊断慢性乙型重型肝炎有重要的临床意义。

Objective To study the clinical value of combined analysis of the serum levels of CA125 and CA199 in the patients suffering chronic severe hepatitis B.Method Serum levels of CA125 and CA199 in 30 patients with Severe chronic hepatitis B,30 patients with benign liver disease were measured by Electrochemiluminescenec immuno assay（ECLIA）.The patients with healthy subjects were served as controls.Results The levels of serum CA125 and CA199 in the Severe chronic hepatitis B group were signifi cantly higher than those in the benign liver disease group and healthy group（P〈0.01）.There was no signifi cant difference in the levels of CA125 and CA199 between the healthy group and the benign liver disease group（P〈0.05）.The sensitivity of CA125,and CA199 in the Severe chronic hepatitis B group was 62.0%,and 66.0% respectively.In the combined analysis of CA125 and CA199,the sensitivity was 84.0%.The sensitivity of combined analysis is significantly higher than that of separate analysis.Conclusion Combined analysis of CA125 and CA199 greatly increase the sensitivity of diagnosis of chronic severe hepatitis B.This can not be the characteristic index of severe hepatitis B,but has great signifi cance in the early clinical diagnoses of chronic severe hepatitis B.
