斑点追踪技术评价急性前壁心肌梗死内外膜圆周应变 被引量:4

Evaluation of subendocardial and subepicardial circumferential strain in acute anterior myocardial infarction with speckle tracking technology

摘要 目的:应用二维斑点追踪技术评价急性前壁心肌梗死心肌内外膜圆周应变(CS)变化规律。方法:对24例正常人和27例急性前壁心肌梗死患者应用二维斑点追踪技术测量其心肌内膜和外膜的最大CS。结果:正常组内膜最大CS明显大于外膜最大CS(P<0.01),内膜CS在前壁心肌梗死相应各节段相对于正常组均减低(P<0.01),而外膜CS仅部分节段减低(P<0.05),将心肌梗死组按肌钙蛋白I(TNI)水平分成两组,TNI>1ng/ml组其基底段、中间段和心尖段平均外膜CS较TNI<1ng/ml组显著降低(P<0.05)。结论:内膜CS是较早发现心肌缺血的一个敏感指标,外膜CS是了解心肌梗死是否是透壁性的一个指标。
