胃镜、腹腔镜联合应用切除胃间质瘤37例临床分析

Combined laparoscopic and endoscopic treatment of GIST of the stomach in 37 patients

目的:评价胃镜与腹腔镜联合应用微创外科治疗胃间质瘤的临床价值。方法:回顾性分析2009年12月-2010年12月中国医科大学附属盛京医院胃肠外科进行经腹腔镜行包括肿瘤在内的胃部分切除术治疗的37例胃间质瘤的临床和病理资料。结果:37例病人行腹腔镜下胃楔形切除术,平均手术时间25min-60min(平均35.5±10.3min),术中出血量20ml-50ml(平均30.0±17.1ml),术后平均住院日(6.5±1.8)d。无并发症出现,平均住院时间6.5d。病理结果:极低风险12例,低度风险22例,中度风险3例,高度风险0例。随访1-12个月,无复发及转移。结论:对于直径小于2cm的胃间质瘤,胃镜和腹腔镜联合应用治疗胃间质瘤是安全可行的。

Objective：To evaluate the feasibility and security of laparoscopy combined with gastroscopy for treating gastic stromal tumors.Methods： From December 2009 to December 2010,37 patients with gastrointestinal stromal tumor of stomach received treatment of laparoscopy combined with gastroscopy.Results： Operation was completed successfully within 25-60 min in all 37 patients,the mean operative time was（35.5±10.3）min,blood loss ranging from 20 ml to 50 ml,the mean postoperative hospital stay was 6.5 days,no postoperative complication occurred.Pathologic analysis of 37 resected specimens showed： extremely low risk 12 cases,low risk 22 cases.No case of recurrence or metastasis was found during a follow-up period from 1 to 12 months.Conclusion： Laparoscopy combined with gastroscopy for treating gastic stromal tumors which are less than 2cm is both feasible and safe,more minimal invasive with a favorable short-term outcome.
