Monitoring Results of Intestinal Pathogens in Xicheng District of Beijing City in 2010

摘要 目的监测北京市西城区感染性腹泻发病情况,分析主要病原菌种类变化及耐药趋势,为感染性腹泻的流行病学研究及临床合理用药提供依据。方法参照中华人民共和国卫生行业标准WS 287-2008、WS 271-2007、WS 289-2008进行病原菌分离、鉴定。参照美国临床和实验室标准协会(Clinical and Laboratory Standards Institute,CLSI)推荐的Kirby-Bauer法进行药物敏感性试验。结果监测结果显示,8—10月份为腹泻病高发期,308份标本中,检出病原菌39株,检出阳性率为12.66%。以弧菌属为优势菌群,其次为沙门菌和志贺菌,志贺菌以宋内血清型为主。药敏试验结果显示,各菌属对抗生素的敏感性有明显差异,病原菌对青霉素类抗生素耐药较普遍,但对头孢类和喹诺酮类抗生素较敏感。结论该区引起感染性腹泻的病原菌种类较多,并且有年龄、季节的分布特点,有些流行菌型也出现明显变化,应加强日常监测,并根据病原菌的菌型变化和耐药性监测结果合理使用抗生素药物。 [Objective] To monitor the incidence of infectious diarrhea in Xicheng District of Beijing City,to analyze the changes of major strain of pathogens and the trend of drug resistance,and provide the basis for guiding epidemiological study of infectious diarrhea and rational administration in clinic. [Methods]The pathogens were isolated and identified according to the Health Industry Standard of the PRC WS 287-2008,WS 271-2007,WS289-2008.The drug susceptibility test was performed by Kirby-Bauer recommended by US Clinical and Laboratory Standards Institute(CLSI). [Results]The peak incidence of diarrhea appeared during August and October.39 strains of pathogens were isolated from 308 specimens and the positive rate was 12.66%.The Vibrio spp was the dominant flora,followed by Salmonella spp and Shigellae spp in which Shigella sonnei was the major serotype.The drug susceptibility test results showed that there was significant differences in the susceptibility at different bacteria genus.The pathogens were general resistant to penicillins antibiotics,however,they were sensitive to cephalosporins and quinolones. [Conclusion]The infectious diarrhea is caused by different pathogens in Xicheng District,which have obvious distribution features of age and season,and the type of some prevalent bacteria type appears notable change,so it is quite important to strengthen the daily monitoring,and to use rationally antibiotics according to different types of pathogen strains and change of drug resistant of bacteria.
