Study on the Short-term Death Rate and the Prognosis of Basal Ganglia Hemorrhage by YL-1 Type Disposable Intracranial Hematoma Smash Puncture Needle Aspiration Under CT Monitoring

摘要 目的探讨CT监视下YL-1型一次性使用颅内血肿粉碎穿刺针穿刺治疗高血压基底节脑出血的近期死亡率及预后研究。方法选择中国医科大学附属第四医院2006年1月-2011年1月诊断为基底节脑出血患者(25-60 ml)300例,治疗手段:内科保守治疗90例(保守组),硬通道穿刺治疗180例(穿刺组),开颅血肿清除术治疗30例(开颅组),随访3个月,比较三组患者术后死亡率及存活者日常生活能力(ADL)。结果穿刺组术后死亡率(9.44%)显著低于保守组(26.67%)和开颅组(23.33%)(P〈0.05);穿刺组ADL1-3/ADL1-5%(85.28%)显著优于保守组(54.55%)和开颅组(65.22%)(P〈0.05)。结论 CT监视下YL-1型一次性使用颅内血肿粉碎穿刺针穿刺治疗高血压脑出血是一种简单快捷、定位准确、创伤小的手术方式,术后死亡率低,显著提高了存活者生活质量。 Objective To investigate the short-term death rate and the prognosis of basal ganglia hemorrhage by YL-1 type disposable intracranial hematoma smash puncture needle aspiration under CT monitoring. Methods The clinical data of 300 patients with basal ganslia hemorrhage(the volume of hemorrhage: 25~60 ml) who were in accordance with the inclusion criteria hospitalized in the Fourth Affiliated Hospital of China Medical University from January 2006 to January 2011 were retrospectively analyzed.90 patients accepted conservative therapy(conservative group),180 patients accepted the transcranial puncture under CT locating(puncture group),and 30 patients accepted traditional craniectomy(craniectomy group).We analyzed the three-month death rate and the prognosis among the three groups. Results The postoperative death rate of puncture group(9.44%) was significantly lower than those of conservative group(26.67%) and craniectomy group(23.33%),(P〈0.05).ADL1-3/ADL1-5% level evaluation of puncture group(85.28%) was significantly better than those of conservative group(54.55%) and craniectomy group(65.22%),(P〈0.05). Conclusions YL-1 type disposable intracranial hematoma smash puncture needle aspiration under CT monitoring is a simple and accurate positioning operation method,with little trauma and lower short-term mortality.It significantly improves the survivor's quality of life.
