摘要
目的:探讨Lorenz散点图在动态心电图诊断及心率变异诊断中的价值。方法:将520例24 h动态心电图数据做成的Lorenz散点图图形与动态心电图诊断及心率变异(HRV)线性指标对照分析。结果:心律失常频次<100次/24 h共203例,Lorenz散点图成一分布图形162例(79.80%),心律失常频次>100次/24 h共317例,Lorenz散点图成多分布图形246例(77.60%),两者分布P<0.005为差异有统计学意义,并且不同类型心律失常的Lorenz散点图有其特征性。在HRV检测中,非线性分析Lorenz散点图与线性频域时域分析对比。HRV正常396例,Lorenz散点图呈"彗星状"(棒球拍状)278例,占70.20%,HRV时域降低者124例,呈异常梭形、棒状、鱼雷状、扇形、复杂图形共89例,异常率71.77%。结论:Lorenz散点图图形特点与动态心电图心律失常类型,与HRV线性时域指标有较好的相关性。
Objective:To investigate the value of Lorenz plot in the diagnosis of heart rate variability using.Methods:Comparative Analysis of Lorenz plot according to 24-hour ambulatory electrocardiogram data with Holter diagnosis and Heart Rate Variability(HRV) linear graphs in 520 cases.Results:Arrhythmia frequency 100 times/24 h was found in 203 cases and Lorenz plot shows as single graphic in 162 cases(79.80%);Arrhythmia frequency 100 times/24 h was found in 317 cases and Lorenz plot shows as varied graphic in 246 cases(77.60%).Their scatter had statistically significant differences in two group P0.005,and Lorenz plot had characteristic according to different types of arrhythmia.In the detection of HRV,comparing nonlinear Lorenz plot with linear algorithms in time and frequency domain,there are 396 cases that show regular HRV,and Lorenz plot showed comet shape(Baseball bat shape) in 278 cases(70.20%),and in 124 cases with low time-domain HRV.89 cases manifest complicated shapes such as spindle,rod,torpedo-shaped,fan-shaped and other varied shapes the abnormal rate was 71.77%.Conclusion:The features of Lorenz plot have certain relevance with arrhythmia type of Armbulatory electrocardiogram and linear time domain indexes HRV.
