小角度集合不足型外斜视的手术治疗

Surgical correction for exotropia of convergence insufficiency pattern by the resection of only one medical rectus

目的探讨小角度集合不足型外斜视的手术方法和疗效。方法取28例集合不足型外斜视，看远斜视角：正位—-10^△，看近斜视角：-15^△—-30^△，以看近斜视角为依据行单根内直肌缩短术，定期随访术后眼位。结果术后正位率，出院时100％，3个月时92．8％，6个月时89．2％，1年时84．2％，2年以后81．8％。术后轻度过矫者，远期发生外斜的比例小且欠矫斜视角度小。结论单根内直肌缩短术矫正小角度集合不足型外斜视，可能获得满意的效果。

Objective To investigate long-time outcome of surgical correction for exotropia of small angle convergence insufficiency pattern by the resection of only one medical rectus. Methods The eye alignment of 28 cases which had been resected only one medical rectus, based on angle at near, were observed regularly. Results The rate of orthotropia were 100% at discharge, 92.8%, 89.2%, 84.2%, 81.8%, at 1 month, 3 months, 6 months, 1 year, 2 years respectively. The possibility and angle of recurrence were deduced greatly by deliberate overcorrection. Conclusions It can be desirable to resect only one medical rectus to correct small angle exotropia of convergence insufficiency pattern.
