摘要
目的分析心房颤动患者循环血浆醛固酮(Ald)水平和心房肌组织中醛固酮合成酶(CYP11B2)的mRNA表达情况,探讨其在心房颤动发病中的作用。方法 38例因心脏疾患住院接受开胸手术的患者,分为窦性心律(SR)组,阵发房颤(pAF)组和慢性房颤(cAF)组。放射免疫方法测量循环血浆醛固酮水平,半定量聚合酶链式反应技术测定CYP11B2 mRNA在心房肌组织中的表达情况。结果 cAF组血浆Ald浓度明显高于pAF组与SR组(P<0.01);pAF组CYP11B2/GAPDH灰度比值较SR组明显升高(P<0.01),较cAF组明显降低(P<0.05)。循环血浆Ald浓度和心房肌组织CYP11B2mRNA表达水平分别与左心房直径变化具有明显的正相关关系(P<0.01)。结论 Ald可能通过介导心房结构重构,参与心房颤动的发生和维持。
Objective To explore the plasma level of aldosterone(Ald) and the tissue mRNA expression of aldosterone-synthase(CYP11B2) in patients with or without atrial fibrillation(AF).Methods Thirty-eight patients who underwent opened thoracic surgery were enrolled.They were divided into 3 groups according to with or without history of AF: Sinus Rhythm group,Paroxysmal AF group and Chronic AF group.The plasma concentrations of Ald were determined by the method of radio-immunity assay,and the mRNA expression of CYP11B2 in atrial myocardium was determined by using Semi-Polymerase Chain Reaction(Semi-PCR).Results Plasma Ald levels were significantly higher in cAF group than pAF group and SR group(P0.01).CYP11B2/GAPDH gray ratio was significantly higher in pAF group than the SR group(P0.01),significantly lower than cAF group(P0.05).Plasma Ald circulating concentration and expression of atrial tissue CYP11B2mRNA and left atrial diameter were significantly correlated with changes in the relationship(P0.01).Conclusion Ald may probably participate in the development and persistent of atrial fibrillation through mediating atrial structural remodeling.
