纤维支气管镜在儿童胸部手术术后肺不张治疗中的应用

The application of bronchoscopy in the treatment of postoperative atelectasis in children with thoracic surgery

目的探讨纤维支气管镜在儿童胸部手术术后肺不张治疗中的临床应用。方法 2008年1月~2010年11月采用纤支镜冲洗治疗为主对20例胸部术后并发肺不张的患儿进行治疗,非纤支镜治疗组采用抗炎、雾化吸入、拍背排痰等常规治疗方法进行治疗。2组患儿均在治疗后1周内复查全胸片或胸部CT,并与治疗前的胸片或者胸部CT比较。结果纤支镜治疗组治疗后1周内有18例患儿肺完全复张,2例部分复张。非纤支镜治疗组治疗后1周内有6例肺完全复张,8例部分复张,6例无改变。结论纤支镜治疗儿童胸部手术术后肺不张操作方法简便,效果满意,值得临床推广。

Objective To investigate clinical application of bronchoscopy in the treatment of postoperative atelectasis in children with thoracic surgery.Methods Twenty cases with chest postoperative atelectasis in children were treated with bronchoscopy from January in 2008 to November in 2010,and non-bronchoscopy group was treated with anti-inflammatory,inhalation,shot back expectoration and other conventional treatment methods.Two groups of patients were reviewed within one week after treatment.Chest X-ray or chest CT was performed,and was compared with pre-treatment.Results Eighteen cases completed lung reexpansion,and 2 partial reexpansion in bronchoscope group within one week after treatment.Six cases had complete pulmonary reexpansion,8 cases had partial re-expansion,and no change occurred in 6 cases in the non-bronchoscope group within one week after treatment.Conclusion The method of bronchoscopy in the treatment of children with atelectasis after chest surgery operation is simple,satisfactory,and worthy of clinical promotion.
