我院静脉用药调配中心不合理医嘱分析及用药干预

Analysis of Irrational Medical Order in Pharmacy Intravenous Admixture Service of Our Hospital and Intervention of Drug Use

目的:分析我院静脉用药调配中心(PIVAS)不合理用药医嘱。方法:调查我院2009年2月-2010年2月PIVAS63562条用药医嘱中药师所记录的不合理医嘱206条,分析不合理用药情况及药师采取的干预措施。结果与结论:经医师更正或终止的医嘱有203条,干预有效率达98.54%。PIVAS药师审核医嘱并对发现的不合理医嘱采取有效的干预,可提高临床合理用药水平。

OBJECTIVE： To analyze irrational medical order in Pharmacy Intravenous Admixture Service （PIVAS）of our hospital. METHODS： 206 irrational medical orders among 63 562 medical orders of PIVAS in our hospital during Feb. 2009 to Feb. 2010 were investigated. Irrational use of drugs and the intervention measures which taken by the pharmacists were analyzed. RESULTSCONCLUSION： By effective intervention of pharmacists, 203 unreasonable medical orders were corrected or discontinued by physicians, accounting for 98.54%. PIVAS pharmacists can improve the rationality of clinical drugs use by checking prescription and taking effective intervention to those unreasonable medical orders.
