纳洛酮辅助治疗乙型脑炎34例临床观察

Thirty-four Clinic Observations on Adjunctive Treatment of Type-B Encephalitis by Adding Naloxone

目的:观察和评价联用纳洛酮治疗乙型脑炎的疗效和安全性。方法:将我院感染科2008年7月～2010年10月收治的67例乙型脑炎患儿随机分成治疗组(34例)和对照组(33例)。治疗组患儿在常规综合治疗的基础上,应用纳洛酮每次静脉输注0.02mg/kg,每天2～3次,连续治疗5d。对照组患儿采用常规综合方法治疗(抗病毒治疗、对症、支持治疗)。结果:治疗组患儿平均退热时间、抽搐停止时间及意识障碍持续时间较对照组明显缩短,对两组患儿随访半年结果显示,治疗组患儿死亡率及后遗症发生率低于对照组。两组比较,差异有显著意义(P<0.05)。结论:联合应用纳洛酮治疗乙型脑炎,提高疗效,减少病残率、降低病死率,为目前治疗乙型脑炎的有效措施,值得临床推广。

Objective：To investigate and evaluate the efficacy and safety of a treatment of type-B encephalitis by adding naloxone, Methods：67 type-B-encephalitis-infected children in the infection department of our hospital from July 2008 to October 2010 were randomly divided into two groups：treatment one （34 cases） and control one（33 case）.On the basis of routine and comprehensive treatment, the children in the treatment group were injected naloxone 0.02 mg/kg per intravenous infusion twice or triple daily.And the treatments lasted five days,As acomparison,the children in the control group were treated merely by routine and comprehensive way （such as antiviral therapy, symptomatic therapy and supportive therapy）.Results：The average time of defervesccncc.cease of convulsion,and disturbance of consciousness of the children in the treamtment group was significantly shortened when comparing with that in the control group.Based on the results of half-year follow-up visits,it was shown that the rates of death and sequela of the children in the treatment group was lower than those in the control group.The discrepancy of the data of the two groups was significant （P〈0,05）.Conclusien：The combined usage of naloxone is an effective measure to treat type.B encephalitis currently, which can enhance the efficacy, reduce the disability rate,and lower the death rate.lt is worthy of clinic promotion.
