摘要
目的:探讨血清T3 T4值对甲状腺毒症的诊断价值,从甲状腺毒症的发病机制分析提供参考。方法:采用电化学发光免疫学方法检测40例甲状腺毒症、60例良性甲状腺病变和40例正常人对照血清甲状腺激素水平,并把甲状腺激素水平与甲状腺毒症临床病理参数进行对比分析。结果:T3、FT4在甲状腺毒症组、甲状腺良性病变组和对照组表达的差异有统计学意义(P<0.05),其中甲状腺毒症组FT3、FT4水平处于较低水平,二者均明显低于甲状腺良性病变组和对照组(P<0.05)。而T3和T4水平在三组之间比较无统计学意义(P>0.05)。甲状腺毒症组血清FT3水平与肿瘤病理分期和淋巴结转移呈负相关(P<0.05),与肿瘤大小无关(P>0.05)。而甲状腺毒症患者血清FT4水平与性肿瘤大小、肿瘤病理分期和淋巴结转移均无关(P>0.05)。结论:甲状腺激素T3水平低下与甲状腺毒症相关,有可能参与了甲状腺毒症的发生发展。而T4不足可能是甲状腺毒症T3水平低下的一个机制。
Objective:To explore diagnostic value of serum T3/T4 value in thyrotoxicosis for the reference by analysis of its nosogenesis.Methods:Serum thyroid hormone levels of 40 patients with thyrotoxicosis,60 patients with benign thyroid tumour and 40 normal persons were compared by electrochemiluminescence immunoassay;serum thyroid hormone levels and thyrotoxicosis clinical pathological parameters were compared for analysis.Results:Thyrotoxicosis Treatment Group,Benign Thyroid Tumor Treatment Group and Control Group showed difference of statistical significance in T3 and FT4(P<0.05).Of the total,Thyrotoxicosis Treatment Group showed significantly lower FT3 and FT4 levels than Benign Thyroid Tumor Treatment Group and Control Group(P<0.05).Comparison of T3/T4 levels among the 3 groups have no statistical significance(P>0.05).Serum FT3 level of Thyrotoxicosis Treatment Group was negatively correlated to tumour pathological stage and lymphatic metastasis(P<0.05),but not correlated to tumor size(P>0.05).However, Serum FT4 level of Thyrotoxicosis Treatment Group was not correlated to tumour size,tumour pathological stage or lymphatic metastasis (P>0.05).Conclusion:Low T3 level of thyroid hormone is correlated to thyrotoxicosis.The hormone perhaps involves in thyrotoxicosis occurrence and growth.And T4 insufficiency is perhaps a mechanism of low T3 level of thyrotoxicosis.
