摘要
目的观察HBIG中和HBsAg的体外与新生儿体内能力,提高出生HBsAg阳性新生儿的治愈率。方法体外测试HBIG中和HBsAg的能力。观察42例出生静脉血HBsAg阴性新生儿使用HBIG 200 IU第4~7天体内的抗-HBs量。将出生HBsAg阳性新生儿分为HBsAg≤40 ng组(Ⅰ组)46例,〉40 ng组(Ⅱ组)17例。比较不同感染量体内HBIG中和HBsAg的能力。结果使用200 IU HBIG 4~7 d后在新生儿体内平均抗-HBs量为226.4 IU。体外试验1 IU HBIG中和HBsAg 42 ng。新生儿体内1 IU HBIG中和HBsAg:Ⅰ组2.04 ng,Ⅱ组1.87 ng。体内外中和能力相差20.5~22.4倍。结论体内外HBIG中和HBsAg能力存在差异,提示新生儿感染HBsAg在组织与血液中分布可能不同。
Objective To observe the HBsAg neutralization capability of hepatitis B immune globulin(HBIG) in vitro test and in vivo among newborn infants.Methods A group of 63 infants born to HBsAg-positive mothers were successfully immunized with HBV vaccine and HBIG.Forty-three of the infants had serum HBsAg less than or equal to 40 ng/ml(group Ⅰ) and 17 of the cases had serum HBsAg greater than 40 ng/ml(group Ⅱ).The concentration of anti-HBs was evaluated on day 4-7 after the application of 200 IU HBIG.The HBsAg neutralization capability of HBIG in vivo was compared the capability in vitro.Results The average amount of anti-HBs was 226.4 IU in newborn infants on day 4-7 after the application and HBIG and HBV vaccine.One IU HBIG neutralized 42 ng HBsAg in vitro,while 1 IU neutralized only 2.04 ng and 1.87 ng HBsAg for the infants of Group Ⅰ and Group Ⅱ in vivo as evaluated,respectively.The HBsAg neutralization capability of HBIG was around 20 times higher in vitro than in vivo.Conclusions The HBsAg neutralization capability of HBIG was different in vitro test vs in vivo evaluation.HBsAg levels in blood and other tissues may difference.
