徐州市健康体检者高尿酸血症流行现况及危险因素分析

Prevalence status and risk factors analysis of hyperuricacidemia among physical examination samples of Xuzhou area

目的通过回顾性分析徐州市健康体检者高尿酸血症的流行现况及相关危险因素，为本地区高尿酸血症的防治提供决策依据。方法选择徐州市中心医院体检中心数据库2006～2009年完成的有效体检资料77355份，根据高尿酸血症诊断结果，使用卡方检验、t检验、Logistic回归等方法进行现况描述和相关因素研究。结果徐州市健康体检人群中高尿酸血症患者5222例，高尿酸血症的总患病率为6．75％，男性患病率为10．29％，女性患病率为0．65％（x2=2651．163，P〈0．001）。女性50岁之前患病率低平，50岁之后持续增加。男性患病率在30岁组、40岁组呈现高峰，50岁组、60岁组降低，60岁后再次快速增加。高血脂、高胆固醇血症、高血压、尿素氮、体重、肌酐、丙氨酸氨基转移酶等7个因素是高尿酸血症的独立危险因素，而高密度脂蛋白增加和女性2个因素是高尿酸血症的保护因素。结论徐州地区健康体检者高尿酸血症患病率较高，男性明显高于女性，两者随年龄增加患病率的变化趋势各有特征。大力推进健康教育，倡导合理膳食和健康生活方式，对降低血尿酸、保护多脏器有重要的临床价值。

Objective To provide the basis for the prevention of hyperuricacidemia through retrospective analysis of the prevalence rate and risk factors of hyperuricacidemia among physical examination samples of Xuzhou area. Methods 77 355 physical examination samples from 2006 - 2009 in Xuzhou area were used to study the prevalence rate and risk factors of hyperuricacidemia by chi - square test, t - test, logistic regression. Results There were 5 222 cases of hyperuricaeidemia, prevalence rate was 6.75% , the rale of male was 10.29% , female 0.65% （ X2 = 2 651. 163, P 〈 0. 001 ）. The rate of female increased after 50 years old group. The rate of male reached the first peak at 30 and 40 years old group, then reduced, and increased after 60 years old group. Hyperlipemia, hypercholesterinemia, hypertension, urea nitrogen, body weight, ereatinine, glutamate - pyruvate transaminase were independent risk factors of hyperuricacidemia. High density fipoprotein and female were independent protect factors of hyperuricacidemia. Conclusions The prevalence rate of hyperuricacidemia was high, and the rate of male was significantly higher than that of female. The trend by age were different between male and female. Health education should be carry out to the people to take healthy diet and healthy life style. That is valueable to reduce the hyperuricacidemia and to protect our organs.
