项丛刺疗法对脑梗死吞咽障碍患者TCD的影响

The influence of treating stroke dysphagia on patients' by item cluster-needling therapy

目的:观察项丛刺疗法对脑梗死吞咽障碍患者TCD的影响,探讨项丛刺疗法的作用机制。方法:将80例脑梗死吞咽障碍患者随机分为项丛刺组、对照组,项丛刺组在药物治疗基础上,取风池、天柱、完骨、廉泉、旁廉泉、金津、玉液、风池、天柱、完骨震颤进针,针向喉结,入1.5～2寸,施以约100转/分速度捻转手法,各约15s,留针30min,期间施手法2次,廉泉,旁廉泉针向舌根直刺2寸,施以100转/分速度捻转手法,各约15s后出针,针感强烈,金津,玉液点刺放血(3天1次)。对照组单纯药物治疗。结果:项丛刺组的总有效率达85%,对照组总有效率77.5%。愈显率项丛刺组为70%,对照组为35%,经统计学处理,P<0.05,两组有显著差异,可见项丛刺组的疗效明显高于对照组。从TCD变化看,即经治疗后两组均有明显变化,经统计学处理,P<0.05,组内、组间均有显著差异,提示项丛刺疗法可以调节脑梗塞,增加脑梗死病灶局部脑血流量,改善脑缺血状态。结论:项丛刺疗法可以调节脑梗塞TCD,增加脑梗死病灶局部脑血流量,改善脑缺血状态,是项丛刺疗法治疗脑梗塞吞咽障碍的作用机制之一。

Objective：To observe key cluster-needling therapy on stroke patients with dysphagia TCD,and to explore key mechanism of cluster-needling therapy.Methods：80 patients with cerebral infarction patients with dysphagia were randomly divided into items cluster-needling group and the control group,item cluster-needling group on the basis of drug therapy,taking the wind pool,Tianzhu,complete bone,spring clean,next to the spring clean,Kim Jin,Yu Ye,the wind pool,Tianzhu,complete bone tremor of the needle,the needle to the Adam＇s apple,into 1.5～2 inches,to impose some 100 r/min speed twisting tactics,each about 15 seconds,the needle 30min,2 times during the approach applied,spring clean,spring clean next to the base of the tongue piercing needle 2 inch,to impose 100 rev/min speed twisting tactics,each about 15 seconds after the needle,needles feeling strong,Kim Jin,Yu Ye prick blood （one time 3 days）.The control group were drug treatment.Results：Items cluster-needling group,the total effective rate was 85% in the control group,the total efficiency of 77.5%.Item cluster-needling group markedly effective rate was 70%,35% in the control group,the statistical treatment,P 0.05,significant difference between the two groups,showing the efficacy item cluster-needling group was significantly higher.Change from TCD to see that in both groups after treatment significantly changes,the statistical treatment,P 0.05,group,there were significant differences between groups,suggesting that items can be adjusted cluster-needling therapy of cerebral infarction,cerebral lesions increase the local cerebral blood flow,improve cerebral ischemia.Conclusion：The items cluster-needling therapy can be adjusted infarction TCD,cerebral lesions increased regional cerebral blood flow,improve cerebral ischemia,is a key cluster-needling therapy for cerebral infarction,one of the mechanisms of dysphagia.
