摘要 目的:探讨resistin在胰腺导管腺癌组织中的表达与临床意义。方法:采用免疫组化法检测45例胰腺导管腺癌组织中resistin的表达,结合临床病理特征进行分析。结果:resistin在胰腺导管腺癌组织中的阳性率为48.9%。高、中及低分化导管腺癌病人中的resistin阳性率分别为25.0%、39.3%及100%,有统计学差异(P<0.05)。按日本胰腺学会JPS分期,Ⅲ~Ⅳ期病人的resistin阳性率为66.7%,显著高于Ⅰ~Ⅱ期病人的22.2%(P<0.01)。resistin阳性病人术后无病生存期短于resistin阴性病人,差异有统计学意义(P<0.05)。Cox多因素分析表明,resistin表达阳性与肿瘤低分化是反映胰腺导管腺癌病人术后无病生存期的独立因素。结论:resistin可能与胰腺导管腺癌的发生、发展有关,其确切的分子生物学机制有待进一步研究。 Objective To study the expression of resistin in pancreatic ductal adenocarcinoma,and analyze its clinical significance.Methods Resistin expression was detected by immunohistochemistry in surgically resected specimens from 45 patients with pancreatic ductal adenocarcinoma,and the results were analyzed with clinical and pathologic characteristics.Results The frequency of resistin expression was 48.9% in pancreatic ductal adenocarcinoma.The expression rate of resistin was 25.0% in well-differentiated tumors,39.3% in moderately-differentiated tumors,and 100% in poorly-differentiated tumors,indicating a significant increase with the decrease in degrees of differentiation(P0.05).The frequency of resistin expression increased significantly in patients with Japanese Pancreas Society stages Ⅲ~Ⅳ than those of stagesⅠ~Ⅱ(66.7% vs.22.2%,P0.01).The recurrence-free survival was significantly shorter in patients with resistin positive expression compared to negative expression ones(P0.05).Cox multivariate analysis showed that positive expression of resistin and poorly-differentiated tumor were significantly associated with recurrence-free survival.Conclusions Resistin expression may be involved in the development and progression of pancreatic ductal adenocarcinoma;but the molecular-biological mechanism remains to be clarified.
