低位直肠癌经腹前切除术后吻合口漏的发生原因分析及防治

Analysis on the Causes Inducing Stoma Leakage Following the via Abdomen Anterior Resection for Low Rectal Carcinoma,and Prevention ＆ Treatment of it

为探讨低位直肠癌经腹前切除术（Dixon术）后吻合口漏的发生原因、预防及治疗，回顾分析我院2007年1月至2010年1月利用双吻合器行低位直肠癌Dixon术的183例患者资料。结果显示，发生吻合口漏15例，其中11例经保守治疗治愈，4例经二次手术治愈。结果表明，吻合口漏是低位直肠癌Dixon术后常见的并发症，小的瘘口可经充分引流、营养支持、抗感染治愈，大的瘘口则需行结肠造口以转流粪便，待瘘VI封闭后再次手术还纳造口。

In order to search for the cause inducing stoma leakage following the via abdomen anterior resection （Dixon procedure） for low rectal carcinoma and its prevention ＆ treatment measures,authors analysed the clinical data of 183 patients who had been subjected to Dixon procedure with double-stapler for their low rectal carcinoma from Jap. 2007 yr. to Jap. 2010 yr. As results,stoma leakage occured in 15 cases,ah mong them 11 cases were cured through conservative therapy, another 4 cases through secondary operation. Results show that stoma leakage is a common complication following Dixon procedure for low rectal carcinoma,in which smaller fistula orifice could be cured through enough drainage, nutrition support and antiseptic treatment;while larger orifice must be solved through making colostomy to transfer feces, and performing secondary procedure to return colostomied orifice after fistula orifice is closed.
