摘要
目的:探讨不同程度单纯性肥胖症儿童体内锌水平和胰岛素抵抗(IR)指数是否存在差异及二者是否存在相关性。方法:在前来儿童保健科检查就诊的学龄前儿童中,抽取体质量正常儿童及轻、中和重度肥胖儿童各20名,检测其血清锌、空腹血糖和空腹胰岛素,计算IR指数,并评价血清锌和IR指数的相关性。结果:轻度肥胖组血锌水平和IR指数与正常对照组无统计学差异(P>0.05),而血锌水平和IR指数在轻度肥胖组(r=-0.36,P>0.05)和正常对照组(r=-0.39,P>0.05)间也不存在相关性。但中、重度肥胖儿童组血锌水平较正常对照组偏低(P值均<0.01),IR指数较正常对照组偏高(P值均<0.01),且这两组中二者均存在负相关(r分别为-0.53和-0.67,P<0.05和<0.01)。结论:中、重度肥胖儿童血锌水平较正常偏低,IR指数较正常偏高,且在中、重度肥胖儿童血锌水平越低,IR指数越高,越易发生胰岛素抵抗。中、重度肥胖儿童应适度增加锌摄入,改善体内血锌偏低状况。
Objective: To study the zinc levels and homeostasis model assessment insulin resistance(HoMA-IR) in different degree obese children and to explore the relationship between zinc level and insulin resistance(IR).Methods: There were four groups(healthy children group,lightly obesity group,middle rank obesity group and heavily obesity group)in the experiment,and n=20 in each group.The zinc level,blood glucose and blood insulin were measured,and insulin resistance was assessed by HOMA-R,and calculated the correlativity between zinc level and insulin resistance.Results: Both the zinc level and HOMA-IR in lightly obesity group weren′t found significant different to those of healthy groups(P0.05).The values of IR were not significantly correlated with the zinc levels in lightly obesity group(r=-0.36,P0.05)and healthy group(r=-0.39,P0.05).However the zinc levels in middle rank obesity group and heavily obesity group were lower than that in healthy group(P0.01).HOMA-IR in the two groups were higher than that in healthy group(both P0.01).HOMA-IR were negatively correlated with the zinc levels in the two groups(r value was-0.53 and-0.67,P0.05 and P0.01 respectively).Conclusion: Both the zinc level and HOMA-IR in lightly obese children weren′t found significant different to those of healthy children.But the zinc levels in middle rank obese children and heavily obeses children were lower than that in healthy ones.At the same time HOMA-IR in middle rank obese children and heavily obese children were higher than that in healthy ones.To middle rank obeses children and heavily obese ones,when the zinc levels were lower,HOMA-IR were higher and insulin resistance more easily took place.But this correlation wasn′t found in lightly obese children and healthy children.
