高血压病社区综合干预效果评价

Evaluation on the Effect of Community-based Synthetic Intervention on Hypertension

目的探讨社区综合干预措施对高血压病的防治效果。方法采用前瞻性研究方法,将650例高血压病患者分为对照组和干预组各325例,对照组患者就诊时,采用一次健康教育后进行自我测量血压的单一模式,即:常规介绍高血压基本知识,发放健康教育资料1次;对Ⅱ、Ⅲ级高血压患者给予常规降压治疗。干预组患者则给予综合干预,进行多次健康教育、反复强化,包括健康教育、心理干预、合理膳食、戒烟限酒、运动指导、用药指导等。结果经过3年的综合干预,干预组血压控制效果明显优于对照组(P<0.01),高血压知识知晓率明显高于对照组(P<0.05),良好的生活行为明显高于对照组(P<0.01),血脂和血糖异常及肥胖发生率低于对照组(P均<0.05),心脑血管病事件发生率低于对照组(P<0.05)。结论社区综合干预可以强化社区高血压患者的健康意识,提高防治效果,最大限度地降低高血压的危险性和减少心脑血管病事件的发生率。

Objective To evaluate the effect of Methods Perspective study was applied. 650 patients with community-based synthetic intervention on hypertension. hypertension were divided into two groups randomly ,intervention group （ n = 325 ） and control group （ n = 325 ）. The patients in control group were treated with the single mode of health education one time, taking one＇s blood pressure and routine treatment of anti-hypertensive strategies. Then a synthetic intervention was carried out to the intervention group, including health education, mental intervention, rational diet, stopping smoking and limit wine, exercise instruction and rational drug use. Results The perspective follow-up study after three years ：The blood pressure control effect of intervention group was superior to the control group（ P 〈0.01 ）, the awareness rate was higher than that in control group （ P 〈 0.05 ） ; The health behaviors was higher than that of control group（P 〈 0.01 ） ;The incidence rate of blood-fat abnormality, pathoglycemia and obesity in control group were significantly lower than those in control group （ all P 〈 0.05 ）. The incidence of cardiovascular-cerebrovascular events was significantly lower than that in control group （ P 〈 0. 05 ）. Conclusion Community-based synthetic intervention can strengthen the health awareness of community hypertension patients and elevate the prevention and treatments effects, reducing hazards of hypertension and the incidents of cardiovascular events.
