摘要
目的探讨旁置输尿管导管持续顺行灌流在输尿管镜气压弹道碎石术中治疗输尿管结石的临床应用价值。方法回顾2009年8月～2011年5月我院泌尿外科采用旁置输尿管导管持续顺行灌流行输尿管镜气压弹道碎石术治疗输尿管结石126例,术中先将输尿管结石一侧打通,或直接从结石旁边插入4F输尿管导管越过结石向上达输尿管上段或肾盂,保留导管退镜,再从导管旁进镜抵达结石碎石。结果 126例次输尿管结石单次结石清除率93.7%(118/126),平均手术时间为(27.5±8.6)min,术中均未发生输尿管穿孔及输尿管黏膜剥脱等严重并发症。结论旁置输尿管导管持续顺行灌流在输尿管镜气压弹道碎石治疗输尿管结石术中能有效防止结石上移,冲出结石碎片,并提供清晰的手术视野,疗效确切、碎石清石成功率高、并发症少。
Objective To explore the clinical application value of paracalculous ureteral catheter continuous antegrade perfusion for treating ureteral stones in ureteroscope pneumatic lithotripsy. Methods From August 2009 to May 2011, 126 patients with ureteral stones underwent ureteroscope pneumatic lithotripsy using paracalculous ureteral catheter continuous antegrade perfusion technique. During the operation, either one side of the ureteral stone was got through, or a 4F ureteral catheter was directly inserted to reach the pelvis and the upper part of the ureter. Set the catheter and retract the scope. Insert the scope again by the catheter to reach the stone and perform the lithotripsy surgery. Results The pulverization rate was 93.7% （118/126）, while the average operation time was 27.5 ± 8.6 min. Furthermore, no incidence of severe complications including ureteral perforation and ureteral mucosa avulsion was observed. Conclusion Paracaleulous ureteral catheter continuous antegrade perfusion in ureteroscope pneumatic lithotripsy can effectively prevent the stones moving upwards, easily flush out the fewer complications. calculus, along with clear surgical field, high efficacy as well as
