摘要
目的 建立多重PCR方法检测免疫球蛋白(Ig)和T细胞受体(TCR)基因重排,探讨在儿童急性淋巴细胞白血病诊断和鉴别中的作用.方法 参照BIOMED-2协作组制定的Ig和(或)TCR检测方法,对儿童ALL患儿114 例,分别检测患者骨髓单个核细胞的IgH、IgK、TCRG和TCRB基因重排.结果 在105例B系儿童ALL患者中,克隆重排的检出率分别为IgH 73%、IgK34%、TCRG 44%和TCRB 35%,所有B-ALL患者Ig基因重排检出率可达85%;在11例T系ALL患者中,克隆重排的检出率分别为TCRB 82%和TCRG 73%,T-ALL 患者TCR基因重排检出率可达100%.结论 BIOMED-2引物系统可以检测出绝大多数儿童ALL患者的Ig/TCR基因重排,是一种有效的检测工具,值得在临床中推广使用.
Objective To establish a sensitive and effective method for detection of immunoglobulin and T-cell receptor(Ig/TCR) gene rearrangement,and to explore its role in diagnosis and differential diagnosis of childhood acute lymphoblastic leukemia(ALL). Methods DNA samples were obtained from the bone marrow mononuclear cells(MNC).IgH,IgK,TCRB and TCRG gene rearrangements were detected by BIOMED-2 multiplex PCR protocols. Results The clonal immunoglobulin(Ig) rearrangements were found in 105 of childhood B-ALL,73% being IGH,34% IgK,44% TCRG and 35% TCRB.While in T-ALL,clonal TCR rearrangements were found in all of the patients,82% being TCRB and 73% TCRG. Conclusion BIOMED-2 multiplex PCR analysis strategy is a reliable and useful technique in the childhood of all patients and it is worth being used in clinical researches.
