摘要
目的探讨64层螺旋CT扫描多平面重组(MPR)及容积再现(VR)后处理技术对于对于鼻骨骨折的诊断价值。材料和方法回顾经我院64层螺旋CT检查的鼻外伤病人53例,每个病人均经下列过程诊断:先由两名高年资医生判读鼻骨横轴位片及重建冠状位片,作出初步印象诊断并记录,再到到工作站上实行MPR和VR图像同屏点对点对比观察,作出最终诊断。结果 53例鼻外伤病人中初步印象诊断:25例多发骨折,最终有一例不符;初步印象13例鼻骨单发骨折中,最终诊断仅5例符合,其它均不符(分别是:多发骨折5例,二例正常鼻骨孔误诊为骨折,一例鼻骨先天缺损变异误诊为骨折);初步印象7例未见明确异常者,最终诊断鼻骨单发骨折一例,鼻骨下缘先天缺损变异二例,未见异常4例;初步印象诊断未见异常8例,最终诊断7例符合,1例为多发骨折。结论 64层螺旋CT后处理技术中的MPR与VR相结合对于鼻骨骨折的诊断具有非常重要的价值。
Objective To explore the diagnostic value by 64-slice spiral CT and post-processing techniques in the diagnosis of nasalbone fracture.Materials and methods Image data in 53 patients with nasal trauma obtained by Philips 64-slice spiral CT were analyzed retrospectively.all patients were diagnosed by the following process,at first both axial and coronal images produced by reformation in the films were observed and analyzed by two higher seniority doctors,drew a preliminary impression and register it,then comparative observation of point to point between MPR images and VR images were carried out in the same screen,made a final diagnosis.Results of 53 cases,35 patients were diagnosed as multiple fractures at first,1 case was inconsistent finally.13 of 53 were diagnosed as single fractue at first,only 5 cases were consistent,the final diagnosis of the remaining were respectively multiple fractures(n=5),foramen of nasalbone(n=2),normal variation(n=1).7 patients were diagnosed as no explicit exception at first,finally 1 was single fracture,2 were normal variation,4 were no abnormal.8 were no abnormal at first,finally 7 cases were consistent,1 case was multiple fracture.Conclusion It is significant valuable in the diagnosis of nasalbone fractue by combining MPR with VR in the post-processing techniques of 64-slice spiral CT.
