胎盘血管瘤对胎儿影响的超声监测

Ultrasound Monitoring of Placental Chorioangioma for Fetus

目的探讨超声监测胎盘血管瘤对胎儿影响的临床价值。资料与方法回顾性分析7例胎盘血管瘤的临床及超声资料。结果本组胎盘血管瘤瘤体直径均>5cm;6例位于胎盘胎儿面,其中1例在胎盘脐带入口处;3例瘤体血流信号丰富,1例瘤体内见脐血管穿行;羊水过多4例,羊水过少2例;2例胎儿脐动脉舒张末期血流频谱消失,4例胎儿宫内发育迟缓,1例胎儿出现心衰及贫血;4例选择适时剖宫产,1例足月顺产分娩,2例孕中期终止妊娠。结论超声是监测胎盘血管瘤对胎儿影响、及时发现并发症并决定分娩时机的有效手段。

Purpose To explore the effect of placental chorioangioma on fetus with Ultrasonic Monitoring.Materials and Methods Clinical and ultrasonic datas from 7 cases of the placental chorioangiomas were studied retrospectively.Results The diameters of the tumors were all larger than 5cm;6 placental chorioangiomas were located in the fetal surface of placenta,and among them,one was near the umbilical cord insertion;3 cases had abundant blood flow within the tumors,umbilical cord vessel was detectived walking through the placental chorioangioma in one case;there were 4 cases with polyhydramnios and 2 cases with oligohydramnios ;2 cases presented with absent end-diastolic flow in the umbilical artery,4 cases with fetal intrauterine growth retardation,1 case with fetal heart failure and anemia;4 cases were performed caesarean section in an appropriate time,1 fetuse was delivered at term and 2 pregnancies were terminated during midpregnancy.Conclusion Ultrasound is an effective method for monitoring the effect to fetus induced by placental chorioangioma、discovering complications timely and determining of delivery time.
