腹腔镜肝左外叶切除治疗肝左外叶血管瘤的体会

Application of Laparoscopic Hepatic Left Lateral Lobectomy in Treatment of Hepatic Hemangioma

目的探讨腹腔镜肝左外叶切除治疗肝左外叶血管瘤的可行性及方法。方法回顾性分析我科2005年1月至2009年6月9例行腹腔镜肝左外叶切除术治疗肝血管瘤患者,血管瘤直径7.1～9.3 cm,其中血管瘤完全位于肝实质内6例,部分突出于肝表面3例。结果 9例均在腹腔镜下顺利完成手术,术中出血200～600 ml,术后1例出现少量腹水,1例出现腹腔包裹性积液,无严重并发症发生。结论在经选择的病例中腹腔镜肝左外叶切除治疗肝左外叶血管瘤是安全可行的。

Objective This paper deals with feasibility aod procedures of application of laparoscopic hepatic left latefal lobeetomy in treatment of hepatic hemangioma. Method A retrospective analysis of laparoseopic hepatic left lateral lobectomy treatment of nine patients suffering from hepatic hemangiomas,with a diameter ranging from 7.1 to 9.3 era,in our department from January,2005 to June ,2009,is made in this paper. Amongst, hemangiomas of six patients were located completely in the hepatic parenchyma,while in the other three cases,part of them protruded on liver surface. Results Laparoscopic hepatic left lateral lobectomy was applied to the nine patients successfully. Bleeding volume during operation was 200 - 600 ml. After the operation, peritoneal fluid was seen in one case and enterocoelia encapsulated fluid in another, and no severe complication was found in all the cases. Conclusion The paper draws a conclusion that it is secure and viable to apply laparoscopic hepatic left lateral lobectomy in therapy of hepatic hemangioma.
