不同髓内钉治疗胫骨骨折的临床观察 被引量:6

摘要 目的:比较磁力导航髓内钉与普通带锁髓内钉治疗胫骨骨折的临床疗效。方法:采用带锁髓内钉与磁力导航髓内钉两种内固定方法分别治疗胫骨骨折患者35例和50例,比较两种治疗方法的手术时间、手术出血量、远端2枚锁钉锁定成功时间、一次性锁定成功率及骨折愈合时间和并发症等;术后随访12-30个月,采用Johner-Wruh评价方法,比较两种治疗方法的肢体功能恢复情况。结果:磁力导航髓内钉内固定术出血量(60.3±6.6)ml、平均手术时间(96.5士5.0)min、远端2枚锁钉锁定成功时间(6.7±2.0)min、一次性锁定成功率96%;与带锁髓内钉内固定术[(110.0±8.5)ml、(170.6±6.2)min、(12.5±2.2)min、77%]比较,差异均有统计学意义(均P<0。05)。结论:两种内固定方法治疗胫骨骨折都具有较高治愈率和远期治疗效果,磁力导航髓内钉具有操作简单、使用方便、不需要X线透视下锁钉,具有创伤小、恢复快等特点,是治疗胫骨骨折的理想方法。 Objective:To compare the magnetic navigation interlocking nail interlocking nail and general treatment of tibial fracture patients.Methods:interlocking nail interlocking nail with the magnetic navigation are two methods of internal fixation of tibial fractures were treated 35 patients and 50 patients,comparing the two treatments operation time,blood loss,distal locking nail two successful Time,success rate and a one-time lock time to fracture healing,fracture healing time and complications;were followed up for 12-30 months,with Johner-Wruh evaluation method to compare two treatments the recovery of limb function.Results:magnetic navigation intramedullary nail fixation amount of bleeding(60.3 ± 6.6)ml,mean operative time(96.5 ± 5.0)min,distal locking screw lock successful two hours(6.7 ± 2.0)min,the success rate of 96% off lock;with interlocking intramedullary nail fixation[(110.0 ± 8.5)ml,(170.6 ± 6.2)min,(12.5 ± 2.2)min,77%] Comparison,the differences were statistically significant(P <0.05).Conclusion:The two internal fixation of tibial fractures have a high cure rate and long-term treatment,nail magnetic navigation is simple,easy to use,does not require X-ray under the nail,with less trauma and quicker recovery characteristics It is the ideal treatment method for tibial fractures.
