PFNA结合环扎钢丝治疗粗隆下骨折的疗效分析

Effect of proximal femoral nail antirotation and cerclage wires in treatment of subtrochanteric femoral fractures

目的:探讨股骨近端防旋髓内钉(PFNA)结合环扎钢丝治疗股骨粗隆下骨折的疗效。方法:采用PFNA结合环扎钢丝治疗粗隆下骨折患者16例,观察患者的手术时间、术中出血量、骨折愈合时间、并发症及髋关节功能评分等相关数据。结果:手术时间35～105min,平均64min;术中出血量200～650mL,平均350mL;骨折愈合时间9～22周,平均15周;除外2例早期出现髋关节疼痛,其余患者均获满意疗效,无1例出现术中骨折、股骨头切出、内固定物断裂等并发症。结论:PFNA结合环扎钢丝治疗股骨粗隆下骨折具有固定可靠、愈合率高、并发症少等优点。

Objective： To investigate the effect of proximal femoral nail antirotation（PFNA） and cerclage wire on subtrochanteric femoral fractures.Methods： A retrospective study was done on 16 patients with subtrochanteric femoral fractures who were treated with PFNA and cerclage wire.The operation time,operative blood loss,complications incidnece,bone healing time and hip function scores were recorded for evaluating treatment outcomes.Results： The average operative time was 64 min（35-105 min）,mean blood loss was 350 mL（200-650 mL） and averaging bone union time was 15 weeks（9-22 weeks）.All patients obtained satisfactory treatment results,except for 2 patients with hip pain.There was no complication such as cutout or breakage of the implants,or peri-implant fracture.Conclusions： PFNA and cerclage wire is an effective way for subtrochanteric femoral fractures with stable fixation,high rate of bone union and few complications.
