摘要 目的探讨乳腺超声图像报告与数据系统(breast imaging reporting and data system contained ultrasonography,BI-RADS-US)对于乳腺病灶分类的意义。方法回顾性分析2007年—2011年间6 618例女性患者共7 941个经BI-RADS-US分类的乳腺病灶,所有病灶均经活检获得病理结果。结果所有7 941个病灶中,59个病灶检查信息不完全或不满意,余7882个按BI-RADS-US分类的病灶,病理证实良性病灶5 767个,恶性病灶2 174个,BI-RADS-US分类的灵敏度为88.50%,特异度为91.02%,准确性为90.36%,阳性预测值(positive predictive value,PPV)为78.74%,阴性预测值(negative predictive value,NPV)为95.48%。结论 BI-RADS-US分类对乳腺肿块良恶性鉴别具有重要应用价值。 Objective To evaluate the application value of breast imaging reporting and data system contained ultrasonography(BI-RADS-US) in the categorization of breast lesions.Methods Totally,7941 breast lesions in 6618 female patients between 2007 and 2011 were classified according to BI-RADS-US retrospectively.All the breast lesions were diagnosed by a histopathological analysis.Results In all 7941 breast lesions,59 lesions could not be evaluated due to incomplete data.In other 7882 lesions,5767 were confirmed benign and 2174 were confirmed malignant by histopathology.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of BI-RADS-US category were 88.50%,91.02%,90.36%,78.74% and 95.48%.Conclusion BI-RADS-US classification has high application value in the diagnosis of benigh and malignant breast lesions,and it's convenient to the clinical management.
