肺炎嗜衣原体三种不同抗原的临床诊断价值分析

摘要 目的探讨分析肺炎嗜衣原体三种不同抗原在肺炎嗜衣原体感染血清学中的临床诊断价值。方法选取2008年1月~2011年1月笔者所在医院188例经检测确诊为呼吸道感染的患者的血清标本,分别采取间接免疫印迹法(Western-blot)和ELISA法对其免疫反应和免疫原性进行分析,对比不同方法检出阳性率和准确率。结果 ELISA法对三种不同抗原的准确性分别为92.6%(174/188)、94.7%(178/188)、96.8%(182/188)。Cpn0146组的阳性率为38.8%(73/188),对比肺炎嗜衣原体的IgG试剂盒检出的阳性率,二者比较差异有统计学意义(P<0.05);Cpn0147的阳性率41.0%(77/188),对比肺炎嗜衣原体的IgG试剂盒检出的阳性率,二者比较差异有统计学意义(P<0.05);Cpn0308的阳性率41.5%(78/188),对比肺炎嗜衣原体的IgG试剂盒检出的阳性率,二者比较差异无统计学意义(P>0.05)。结论三种不同抗原中,以Cpn0308在肺炎嗜衣原体感染血清学的诊断实验中的检出准确率与阳性率最高,对临床诊断肺炎嗜衣原体有良好的应用价值,值得研究利用。 Objective To investigate the analysis of three different antigens of Chlamydia pneumoniae addicted to Chlamydia pneumoniae in the clinical serological diagnosis. Methods From January 2008 to January 2011 ,our hospital tested 188 patients diagnosed with respiratory tract infection serum samples, indirect Western blot (Western -blot)and ELISA were taken to analyse the immune response and immunogenicity, comparing different methods of positive rate and accuracy. Results The accuracy of three different antigens in ELISA method were 92. 6% ( 174/188 ) ,94. 7% ( 178/188 ), 96. 8 % ( 182/188 ) respectively, the positive rate of Cpn0146 group was 38.8% ( 73/188 ), compared to addicted to Chlamydia pneumoniae IgG positive rate of detection kits, there are significant differences ( P 〈 0. 05), with statistical significance ; CpnO147 positive rate of 41.0% (77/188), compared to the positive rate of IgG,there were statistical significance between them(P 〈 0. 05) ;The positive rate of Cpn0308 was 41.5% (78/188), compared to the positive detection rate of IgG, there was no statistically significant between them ( P 〉 0. 05). Conclusion The Cpn0308's detection accuracy and positive rate was the highest of all, it was worthy of study.
