运动性肌红蛋白尿引起的急性肾功能衰竭 被引量：5

Exertional rhabdomyolysis and myoglobinuria induced acute renal failure

摘要 31岁男性,剧烈运动后即出现少尿型急性肾功能衰竭(ARF),并出现全身肌肉酸痛、高钾血症及血肌酶显著升高、肌红蛋白尿、浓茶色尿。肾活检病理表现为肾小管腔内颗粒管型形成及急性肾小管坏死,经肌红蛋白染色证实肌红蛋白管型形成及肌红蛋白在肾小管上皮细胞沉积。该患者最终诊断为运动性肌红蛋白尿所致的ARF。 A 31-year old man presented with exercise induced acute renal failure. He experienced oliguria, myalgia, hyperkalemia, elevation of blood creatine kinase, myoglobinuria and dark urine. Renal biopsy revealed mild brown granular casts and acute tubular injury. Myoglobin casts and myoglobin deposits in tubular epithelial cells were conformed by immunohistochemistry. The diagnosis of exer/ional rhabdomyolysis and myoglobinuria induced acute renal failure was made.
