逆行胰体尾切除在进展期胃癌手术中的应用

Application of retrograde distal pancreatectomy in the surgery for advanced gastric cancer

目的总结逆行胰体尾切除在进展期胃癌手术中运用的效果。方法回顾分析逆行胰体尾切除术的11例进展期胃癌患者的临床资料。结果 11例均完整切除肿瘤及受累组织,达到R0根治标准。手术时间(3.0±0.5)h,术中出血少;术后恢复良好,无严重并发症。结论在进展期胃癌手术中,逆行胰体尾切除是在胰后间隙分离,易于先断血管及脾胰体尾切除,能保持原位切除肿瘤,避免因挤压引起肿瘤经血管、淋巴管扩散,减少手术出血,缩短手术时间。

Objective To summarize the outcomes of retrograde distal pancreatectomy in the treatment of advanced gastric cancer.Methods The clinical data of 11 patients underwent retrograde distal pancreatectomy were analyzed retrospectively.Results The tumor together with invading organs were wholly dissected in all patients by retrograde distal pancreatectomy and the operations achieved the standard of R0 resection with less blood loss.The surgery took about 3 hours.All patients recovered well and discharged without severe complications.Conclusion In the treatment of advanced gastric cancer,retrograde distal pancreatectomy has the advantages of easily mutilating blood vessel and resecting distal pancreas and spleen,keeping resection of tumor in situ,avoiding tumor compression and metastasis,reducing blood loss and shortening the operation time.
