摘要
替诺福韦酯(TDF)是一种核苷酸类逆转录酶抑制剂,具有良好的抗HIV、HBV作用以及服药方便、耐受性好、药物之间相互作用少等特点,已被批准治疗HIV感染,但上市后观察到可引起肾功能损害,包括血清肌酐水平升高、低磷酸盐血症、肾功能不全、肾功能衰竭和范可尼综合征等。TDF相关肾功能损害不容忽视,本文介绍其机制、临床表现和危险因素等,以期引起重视。
As a nucleotide reverse transcriptase inhibitor,tenofovir disoproxil fumarate(TDF)has been approved for the treatment of HIV infection,which also has good anti-HBV effectiveness.Furthermore,it has advantages of convenient administration,good tolerance,less interactions,etc.However,many postmarketing observations found that TDF is associated with nephrotoxicity,including increase of serum creatinine levels,hypophosphatemia,renal insufficiency or failure and Fanconi syndrome.Therefore,TDF-related renal damage should be alerted.The mechanism and clinical manifestation of TDF-related renal damage and the research progress of the potential risk factors contributed to the TDF related nephrotoxicity are reviewed in this paper.
