乳腺癌改良根治术后皮下积液的预防和处理

Prevention and treatment of subcutaneous liquid accumulation after modified radical mastectomy operation

目的:探讨乳腺癌改良根治术术后皮下积液发生的原因、预防措施及处理方法。方法:回顾性分析行乳腺癌改良根治术的155例患者术后发生皮下积液的情况。结果:155例患者发生皮下积液21例,占13.54.%,发生积液的患者常规给予输液器导管平压引流、BD留置针引流以及弹力绷带加压包扎后均治愈。结论:合理的皮瓣切口设计及皮瓣厚度、术中电刀功率的大小、充分止血、预防淋巴管漏、术后确保负压引流通畅及适当的加压包扎均是预防皮下积液的重要措施。

Objective To investigate the causes,prevention and cure measures of subcutaneous liquid accumulation after modified radical mastectomy operation.Method Total 155 cases with breast cancer underwent modified radical mastectomy in our department was retrospectively analyzed.Results Among 155 cases,21 had subcutaneous liquid accumulation,the incidence was 13.54%,all the patients with subcutaneous liquid accumulation were treated with the compression bandage on chest wall with the continuous drainage or BD detaining needle regularly,and all cured.Conclusion Design of the incision and thickness of the skin flap,power of the electrical knife,effective bleeding prevention,prevention of lymph node leakage,adequate suction drainage and moderate pressure wraps after operation are important to prevent subcutaneous liquid accumulation.
