锁定钢板治疗肱骨近端骨折临床研究

Clinical analysis of locking plate to cure the proximal humerus fracture

目的探讨锁定钢板治疗肱骨近端骨折的临床疗效及并发症发生情况。方法 200例肱骨近端骨折患者平均随机分为锁定钢板组和传统手术组,分别予锁定钢板固定和传统手术固定,术后随访6~12个月,比较两组的疗效和并发症发生情况。结果锁定钢板组疗效优秀率（80%）高于传统手术组（24%）,（χ2=6.44,P〈0.05）,总体治疗效果优于传统手术组（H=8.43,P〈0.05）。随访期间两组出现的并发症包括愈合不良、内固定不良、肱骨头坏死等,锁定钢板组并发症发生率均低于传统手术组,总并发症发生率锁定钢板组也低于传统手术组（P〈0.05）。结论锁定钢板治疗肱骨近端骨折的临床疗效确切,并发症少,有进一步研究推广的价值。

Objective To discuss the clinical effects and complication of the locking plate in the treatment of the proximal humerus. Methods 200 patients suffered from the proximal humerus, were divided into the locking plate group and the traditional operation group at random. The operations were performed with fixing with the locking plate and the traditional method, and then compare the clinical effects and complication of the two groups. Results The excellence rate of the locking plate group （80%） was higher than that of the traditional operation group （22%） , with P0.05 , and the total treatment effect of the locking plate group was better than that of the traditional operation group （P0.05） .The complications rate such as the postoperative blood vessels nerve damage, poor healing poor fixing and the necrosis of humeral head in the locking plate group were all lower than those in the traditional operation group and the total complication rate of the locking plate group was also lower than that of the traditional operation group （P0.05）. Conclusion The method of locking plate is effective to cure the proximal humerus fracture , with little complications.
