CT引导下腰椎间盘突出臭氧髓核消融术88例疗效分析

Evaluation report of CT-guided ozone nucleus ablation effect for 88 patients with lumbar disc herniation

目的评价臭氧（O3）髓核消融术联合神经根周围注射O3治疗腰椎间盘突出症的疗效。方法 88例腰椎间盘突出症患者随机分为两组,在CT引导下,Ⅰ组（n=44）行椎间盘穿刺O3髓核消融术,盘内注射50μg/mlO310～20ml,盘内边缘注射混合液0.5ml,Ⅱ组（n=44）行腰椎间盘穿刺O3消融术方法同Ⅰ组,同时神经根周围注射40μg/mlO310~15ml、混合液1.0ml。两组患者分别在术后1周、3个月、6个月采用双盲法用Oswestry Low Back Pain Disability量表评估疗效。结果术后第1周Ⅰ组和Ⅱ组有效率分别为75%和90.9%;3个月有效率分别为72.7%和93.2%;6个月时分别为70.5%和88.6%。Ⅱ组较Ⅰ组在术后1周、3个月、6个月疗效均显著增高（P〈0.05）。结论 O3髓核消融术联合神经根周围注射O3是治疗腰椎间盘突出症的一种有效、安全的方法,值得推广应用。

Objective To evaluate the clinical value of ozone nucleus ablation combined with perineural root ozone injection for lumbar disk herniation. Methods 88 patients with lumbar disk herniation were randomly divided into two groups.By CT guiding,ozone nucleus ablation was performed with O310～20 ml （50 μg/ml） intradiscal injected in groupⅠ（n=44）, and 0.5 ml mixed liquor was injected to disc edge. In group Ⅱ（n=44）, after the same treatment as that of groupⅠ was performed, O310～15 ml （40 μg/ml） and mixed liquor 1.0 ml were injected around nerve root.Using double-blind method, the curative effect was evaluated at 1 week, 3 months, 6 months after the treatment by Oswestry Low Back Pain Disability Questionnaire. Results The effective rate of groupⅠ and group Ⅱ was 75%and 90.9%, 72.7% and 93.2%, 70.5% and 88.6% after the treatment of 1 week, 3 months and 6 months, respectively, with significant difference between two groups （P0.05）. Conclusion Ozone nucleus ablation combined with perineural root ozone injection is an effective and safe treatment for lumbar disk herniation.
