Curative effect of radiotherapy in patients with brain metastasis from lung cancer

目的对肺癌脑转移全脑放疗局部加量的疗效进行分析。方法回顾性分析51例肺癌脑转移患者全脑放疗局部加量的治疗效果。结果中位总生存时间11个月。单因素分析显示脑转移灶总剂量,颅外转移,RPA分级与总生存有关。多因素分析显示总剂量,颅外转移和RPA分级是独立预后因素。结论全脑放疗局部加量的方式对于RPA分级为1～2级,没有颅外转移的肺癌脑转移患者全脑放疗局部加量的方式在疗效上能更为获益。

Objective To analyze the curative effect of brain metastasis from lung cancer applied with an external beam radiotherapy boost after whole brain radiotherapy. Methods The clinical data of 51 patients with brain metastasis from lung cancer treated sequentially with whole brain therapy and radiotherapy boost, were analyzed retrospectively. Result The median overall survival time was 11 months. On univariate analysis, improved OS was found to be significantly related with total dose of brain metastasis site, extracranial metastasis and RPA. On multivariate analysis, total dose of brain metastasis site, extracranial metastasis and RPA were independent prognostic factors. Conclusions There might be a group of patients with RPA 1-2 and no extracranial disease who may benefit from dose escalation after WBRT to the metastatic site.
