自制简易负压吸引器在治疗糖尿病足溃疡创面中的效果

Effects of Simple Self-made Negative Pressure Under Vac Dressing in Anabrosis of Patients with Diabetic Foot

目的探讨自制简易负压吸引器在治疗糖尿病足溃疡创面中的效果。方法选择2010年4月至2011年5月在第二军医大学长海医院内分泌科和血管外科住院的糖尿病足患者共52例作为研究对象,采用随机数字表法将其分为观察组和对照组,每组26例。对照组患者采用常规伤口敷料进行常规换药,观察组患者采用自制简易负压吸引器进行换药,并比较两组患者7d后创面面积减少情况、14d后总疗效及两组治愈时间。结果治疗前两组患者创面面积的差异无统计学意义。治疗7d后,观察组患者创面面积减少30%以上的达50%,而对照组仅占23.1%,观察组疗效优于对照组,差异有统计学意义(u=2.259,P=0.024)。治疗14d后,观察组患者治愈率为34.6%、显效率为50.0%;而对照组则分别为11.5%和19.2%,对照组患者治疗的有效率明显低于观察组,差异有统计学意义(P<0.01)。观察组治愈时间明显短于对照组,差异有统计学意义(u=2.052,P=0.040)。结论简易负压吸引是一种良好的促进糖尿病足创面愈合的方法,其疗效优于传统方法,值得推广。

Objective To explore the effects of simple self-made negative pressure under VAC dressing in treating anabrosis caused by diabetic foot.Methods From April 2010 to May 2011,52 patients with diabetic foot in the Endocrine Division and Vascular Division in our hospital were randomized into experimental group and control group,with 26 patients in each group.The control group used the conventional wound dressings.While the experimental group used the simple self-made negative pressure under VAC dressing.Comparisons were conducted in terms of the the decreasing wound lesion 7 days later,the overall efficacy 14 days later and the curing time period.Results There was no significant difference on the wound lesion before the treatment between both groups.After 7 d of treatment,the wound lesion reduction by 30% was observed in 50% and 23.1% of patients in the experimental group and the control group,respectively（u=2.259,P=0.024）.After 14 d of treatment,the cure rate and effectual rate in the experimental group and the control group were 34.6%,50.0% and 11.5%,19.2%,respectively.The effectual rate was significantly lower in the control group than in the experimental group（P0.01）.The cure time was significantly less in the experimental group than in the control group（u=2.052,P=0.040）.Conclusion The simple self-made negative pressure under VAC dressing works well in promoting the healing of wound caused by diabetic foot.Since the therapeutic effect was superior to that of the conventional method,the method deserves further popularization.
