Prevention of Infraskin-flap Hydrops Following Radical Mastectomy（521 Cases Analysis）

目的探讨乳腺癌根治术后皮瓣下积液的预防方法。方法对采用预防皮瓣下积液新方法的乳腺癌根治术病例与实施传统乳腺癌根治术病例进行对比研究。结果乳腺癌根治术后皮瓣下积液率由原来的18．1％下降到3．1％．新方法效果显著。结论乳腺癌根治术后预防皮瓣下积液的关键是：防止淋巴管瘘，合理使用电刀．置双引流管并负压吸引．胸带加压包扎。

Objective To investigate the ways of reducing the incidence of infraskin-flap hydrops after radical mastectomy. Methods The patients undergoing radical masteetomy combined with new ~ ：.methods to prevent hydrops were studied and compared with those operated on by conventional procedure. Results The incidence rate of infraskin-flap hydrops after radical ma＇stectomy was reduced from 18.1% to 3.1%. Surely, utilization of new surgical procedure can avoid to a certain degree developing postoperative complication It is very efficacious. Conclusion In order to ensure successful Operations and reduce the complication ,we should pay close attention to the following crucial points during operation ： ①Securely ligating all the lymphatic vessels in the wound and avoiding subaxillary lymphatic leakage . ②Proper use of electrocoagulation. ③Striet hemostasis in the wound. ④A double tube drain connected to a negative pressure suetion appratus used for plenty of drainage .⑤Pressure bandagin~ of the wound.
