结合面部美学单位设计菱形皮瓣修复面部皮肤缺损

Repair of skin defect of the face with rhomboid flap in accordance with facial aesthetic units

目的探讨结合面部美学单位设计菱形皮瓣修复面部皮肤缺损的适应证及术后疗效。方法根据面部美学单位设计菱形皮瓣修复面部皮肤缺损29例。缺损位于下颏区域4例,颊部10例,颞区3例,鼻翼外侧、鼻背侧壁区域9例,内眦区域3例。缺损面积最小者10mm×10mm(鼻翼区),最大者30mm×35mm(下颏区)。修复缺损时,结合面部美学单位应用Limberg皮瓣15例,Dufoumental皮瓣11例,Webster 30度皮瓣3例。结果所有皮瓣均成活。12例患者随访1～24个月,皮瓣色泽质地与周围组织无明显差别,隐藏于面部美学单位分界线、皱纹等部位的瘢痕不明显。结论结合面部美学单位设计菱形皮瓣修复面部皮肤缺损是一种简单易行可靠的技术。

Objective To evaluate the outcomes of repair of skin defect of the face with rhomboid flap in accordance with facial aesthetic units.Methods The skin defect of the face was repared in 29 patients aged 5 to 80 years old with 29 rhomboid flaps according to facial aesthetic units,of whom the defect located on the chin in 3 caess,cheek in 10 cases,nasal alar and dorsum regions in 9 cases,medial canthal in 3 cases,forehead in 3 cases.The smallest defect sized 10 mm×10 mm,and the largest 30 mm×35 mm.Patients were treated by Limberg flap in 15 cases,Dufoumental flap in 11 cases and Webster 30 degrees flap in 3 cases according to facial aesthetic units.Results All flaps survived postoperatively and healed uneventfully.Twelve patients were followed up for 1 to 24 months.The cosmetic results were satisfactory.Conclusion The rhomboid flap in accordance with facial aesthetic units is an effective and reliable technique for the treatment of defect on the face.
