热疗结合胸腔药物灌注治疗恶性胸腔积液58例疗效分析

The effect of drug perfusion chemotherapy combined with high-frequency hyperthermia in the treatment of malignant hydrothorax

目的观察体外高频热疗联合胸腔药物灌注化疗治疗恶性胸水的疗效及不良反应。方法恶性胸水58例,同期分为观察组及对照组,观察组31例,对照组27例。胸腔穿刺行闭式引流干净胸水后,观察组将顺铂(DDP)60～100 mg溶于生理盐水50 ml中注入胸腹腔,嘱患者反复翻身变换体位,使药物与胸膜充分接触,约半小时后即行高频热疗;灌注结束后,采用高频热疗机行胸腔局部热疗42℃～43℃,60分钟,每周热疗1次;对照组给予生理盐水50 ml+60～100 mg胸腔内注入。两组均每周灌注1次,连续灌注3周,评定疗效。结果观察组有效率80.7%;对照组有效率48.2%,两组有效率比较差异有统计学意义(P<0.05)。两组不良反应无明显差异。结论体外高频热疗联合胸腔药物灌注化疗治疗恶性胸水疗效好,可以耐受。

Objective： To observe the efficacy and side effects of high - frequency hyperthermia combined with drug peffusion chemotherapy in the treatment of malignant hydrothorax. Methods： A total of 58 patients with malignant hydrothorax were divided into the treatment group and the control group, with 31 cases in the treatment group and 27 cases in the control group. In the treatment group, after the malignant hydmthorax was drained, normal saline 50ml with cisplatin 50 - 100mg was perfused into pleural cavity with body cavity perfusion therapy machine, and then pleural cavity was heated to 42℃ - 43℃ for 60 minutes, twice a week. The patientsin control group was only given normal saline 50 ml and cisplafin 50- 100mg. For both of the groups, this therapeutic schedule was given once a week for 3 weeks. Results： The effective rate was 80.7% in the treatment group and 48.2% in the control group（ P 〈 0.05 ）. There was no significant difference between the two groups in side effects. Conclusion： High - frequency hyperthermia combined with drug perfusion chemotherapy in the treatment of malignant hydrothorax has good efficacy, and can be tolerated.
