右美托咪定对急性心理应激的免疫保护作用

Immunoprotection of dexmedetomidine from acute psychological stress

[目的]探索右美托咪定在局麻手术中对急性心理应激的免疫保护作用。[方法]择期局麻下行乳腺包块切除术的病人80例,随机平分为两组。实验组在局麻的同时复合静脉泵注右美托咪定;对照组只行局麻。两组病人均于术前及术毕时抽血检测CD4、CD8,行VAS疼痛评分、SCL-90心理评估,随访术后并发症。[结果]右美托咪定组术后SCL-90评分(110.15±11.87)及VAS评分(0.33±0.32)低于对照组的(163.17±20.17)和(3.65±0.24),术后CD8(28.11±1.34)低于对照组的(31.25±1.70),CD4/CD8(1.53±0.17)高于对照组的(1.43±0.15),差异有显著性意义(P<0.05)。[结论]右美托咪定对局麻手术中的急性心理应激有免疫保护作用。

[ Objective] To study the immunoprotective effect of dexmedetomidine on acute psychological stress in local anesthesia surgery. [ Methods ] Eighty patients undergoing mammary gland block ectomy were divided into two groups randomly. Experimental group received local anesthesia and dexmedetomidine and control group took local anesthesia only. CD4 and CDs levels were examined in preoperative and postoperative periods, VAS pain score and SCL -90 psychological assessment were performed. Both the two groups of patients received postoperative follow - up for complications. [ Result] Postoperative SCL - 90 grade （ 110.15 ± 11.87 ）, VAS score （0.33 ± 0.32） and the CD8 （ 28.11±1.34） level of experimental group were lower than these in control group （ 163.17 ± 20.17,3.65 ±0.24,31.25± 1.70 ）, while CD4/CD8 （ 1.53 ± 0.17） was higher than that in control group（ 1.43±0.15） （P 〈0.05）. [ Conclusions] Dexmedetomidine can protest the acute psychological stress response by the way of regulating immune function.
