摘要
目的:探讨慢性肾功能不全(chronic renal dysfunction,CRF)患者C-反应蛋白(C-reactive protein,CRP)水平与左室收缩功能损害的相关性研究。方法:将68例CRF患者分为血液透析组(32例)及未行血液透析组(36例),15例健康者为正常对照组,所有观察者均测定CRP水平,并行超声心动图检查,比较各组之间CRP水平、室间隔厚度(IVST)、左室后壁厚度(LVPWT)、射血分数(EF)、左室短轴缩短率(FS)及室壁运动情况。结果:血液透析组及未行血液透析组CRP水平高于正常对照组(P<0.01),血液透析组高于未行血液透析组(P<0.05);血液透析组IVST、LVPWT较未行血液透析组及正常对照组增高(P<0.05),EF、FS较未行血液透析组及正常对照组降低(P<0.05);血液透析组左室壁运动均不同程度欠协调,未行血液透析组及正常对照组左室壁运动协调;血液透析组与未行血液透析组CRP水平与IVST、LVPW呈正相关,与EF、FS呈负相关。结论:慢性肾功能不全患者CRP水平明显增高,与左心功能损害程度具有相关性。
Objective:To study the relationship between the serum hs-CRP level and the damage of left ventricular systolic function in the patient with chronic renal failure(CRF).Methods:68 patients with CRF were allocated to two groups: hemodialysis(32 cases) and non-hemodialysis(34 cases),with 15 healthy adults as the control group.All cases were detected the serum hs-CRP level,and examined IVST,LVPWT,EF,FS and the condition of left ventricular wall motion by echocardiography.Results:The serum hs-CRP level in the hemodialysis and the non-hemodialysis groups was higher than that in the control group(P0.01),and the hemodialysis group′ s was higher than the non-hemodialysis group′s(P0.05).IVST and LVPWT in the hemodialysis group were higher than those in the non-hemodialysis group and the control group(P0.05).EF and FS in the hemodialysis group were lower than those in the non-hemodialysis group and the control group(P0.05).The left ventricular wall motion was less coordinated in varying degrees,but that in other two groups was coordinated.The serum hs-CRP level was positively correlated with IVST and LVPWT,and negatively correlated with EF and FS in the hemodialysis and non-hemodialysis groups.Conclusion:The serum hs-CRP level in the patient with CRF is increased obviously,and positively correlated with the damage degree of left ventricular systolic function.
