妊高征新生儿胆红素水平非病理因素调查

Study on Non-pathological Factors of Neonatal Bilirubin Level Produced by Puerpera with PIH

目的探讨患妊娠期高血压疾病的产妇在围产期时,各种非病理性因素对其新生儿胆红素水平的影响。方法选择患妊娠期高血压疾病的产妇170例,产下新生儿后立即收集新生儿脐带血,采用胆红素测定仪测定新生儿脐带血胆红素水平。按胆红素水平高低将所有患儿分成两组(高胆红素组和胆红素正常组)。记录妊高征产妇在围产期时可能影响新生儿胆红素水平的各种非病理因素,探讨各种因素对两组新生儿的影响。结果妊高征产妇的病情程度、患病时间、孕周、新生儿体质量四项因素对新生儿脐带血血清胆红素水平的影响最显著(P<0.05)。产妇年龄、分娩方式、生产胎次三项因素对新生儿脐带血血清胆红素水平无太大影响(P>0.05)。结论妊高征产妇的病情程度、患病时间、孕周、新生儿体质量四项因素对新生儿血清胆红素水平有重要影响,为提高新生儿质量应对妊高征产妇早期诊断早期治疗。

Objective To discuss non-pathological factors and its effect on neonatal bilirubin level during perinatal period of puerperants with PIH. Methods Selected 170 puerperants with PIH, collected neonatal cord blood immediately after birth and tested bilirubin level with bilirubin analyzer. Divided the children into two groups by bilirubin level （ high-level group and normal group ）. Recorded a variety of non-pathological factors of neonatal bilirubin level, discussed their effect on the two groups. Results The level of illness, sick time, gestational weeks and neonatal weight of puerperants with PIH had the most predominant effect on bilirubin level of neonatal cord blood （ P 〈 0.05 ）. The factors of age, way of delivery, times of delivery had limited effect on bilirubin level of neonatal cord blood （ P 〉 0.05 ）. Conclusion Four factors including level of illness, sick time, gestational weeks and neonatal weight of puerperants with PIH have great effect on bilirubin level of neonatal cord blood, in order to enhance neonatal quality, we should diagnose and treat as early as possible with puerperants with PIH.
