四藤通脉饮治疗颅内动脉瘤术后脑血管痉挛的临床研究

Clinical Study on the Therapeutic Effects of SiTeng Tong MaiYin in Patients with Cerebral Vasospasm of Intracranial Aneurysm Surgery

目的:观察四藤通脉饮治疗颅内动脉瘤术后脑血管痉挛的临床疗效。方法:将42例颅内动脉瘤术后脑血管痉挛的患者随机分为对照组20例和治疗组22例,对照组在术后进行预防感染、脱水、尼莫同等综合治疗,治疗组在综合治疗的基础上加用四藤通脉饮治疗,10天后观察疗效。结果:42例患者治疗后TCD检查,血管痉挛符合率达100%,治疗组第5天、第10天脑血管痉挛缓解程度较对照组明显减轻,2组之间有显著差异(P<0.05),42例患者治疗前后Hunt-Hess分级比较,治疗组较对照组下降明显,有统计学意义(P<0.05),2组治疗前后脑梗死发生率比较,差异有统计学意义(P<0.05)。结论:四藤通脉饮有助于缓解颅内动脉瘤术后脑血管痉挛的程度,降低脑梗死发生率,改善患者预后。

Objective：To observe clinical efficacy ofSiTeng TongMaiYin in patients with cerebral vasospasm of intracranial aneurysm surgery.Method：42 patients with cerebral vasospasm of intracranial aneurysm surgery were ran- domly divided into control group（20 cases） and treatment group （22 cases）. The control group was given the prevention of postoperative infection,dehydration,Nimodipine,etc.The treatment group was given SiTeng TongMaiYin on the basis of comprehensive treatment.The clinical efficacy was observed after 10 days.Result：42 patients received TCD examina- tion,the rate of blood vessel spasm was 100%.The extent of cerebral vasospasm was relieved in the treatment group 5 days and 10 days,and there was significantly different between the two groups （P〈 0.05）.The Hunt-Hess grade of the treatment group was significantly reduced compared with the control group before and after treatment, and there was statistically significant （P〈 0.05）.Compared to the incidence of cerebral infarction before and after treatment between the two groups,there was statistically significant （P〈 0.05）.Conclusion：SiTeng TongMaiYin can relieve the extent of cerebral vasospasm of intracranial aneurysm surgery,reduce the incidence of cerebral infarction and improve prognosis.
