手术时机的选择对急性胆囊炎预后的影响

Effect of Surgical Timing on Outcome of Cholecystectomy for Acute Cholecystitis

目的探讨手术时机的选择对急性胆囊炎患者预后的影响。方法将我院2008年至2011年收治的急性胆囊炎患者128例分为早期手术组(Ⅰ组,n=75)和延期手术组(Ⅱ组,n=53),回顾分析两组手术时间、手术治疗结果、并发症的发生以及住院天数等。结果和延期手术组相比,早期手术组住院天数明显缩短(P<0.01),两组在手术治疗效果、并发症的发生、手术时间等方面无显著差别。结论急性胆囊炎患者早期手术治疗是安全有效的治疗方法,并能显著缩短总住院天数,改善患者预后。

Objective To evaluate the effect of surgical timing on outcome of cholecyatectomy for patients with acute cholecystitis. Methods 128 patients were divided into two groups： Group Ⅰ： pateents treated by early cholecystectomy and Group Ⅱ： patients treated by delayed cholecystectomy. We retrospectively analyzed and ofcomapred the following： operative time, postoperative morbidity and mortality,complictions, hospital stays. Results Out of 128 patients with acute cholecystitis, 75 patients underwent surgery, whereas 53 were treated conservatively. There were no significant difference between the two groups in operative time, postoperative morbidity and mortality, complications; however, the total hospital stays [（11 ±2.3） vs （18 ±2.6） days, P 0.01] signoficantly reduced in patients undergoing early cholecystectomy. Conclusion Early cholecystectomy is a safe, effective treatment of acute cholecystitis, with shorter hospital stays and better outcome for patients.
